HISTOPATHOLOGY GUIDELINES​
HISTOPATHOLOGY REQUEST
All collected specimens (tissues or body fluid) from the patient should be submitted for histopathologic documentation.
Necrotic, burned, cauterized, unfixed, &/or autolyzed specimens are not recommended.

Histopathologic/cytologic request should include:
Printed name of patient, age, sex, civil status
Specimen/s- container labelled specimen with patient’s name , date collected/operation
Specimen-specific source/organ, and operation performed.
Short clinical history and PPE
Clinical impression
Name of requesting physician & clinic/hospital
Relevant laboratory test results, previous biopsy, & imaging procedures
SPECIMEN FIXATION
Pap’s & Tzanck smears
Smear on slide and immediately dip in 95% ethanol for 10 minutes. Air dry & wrap in paper.
Include OB history, LMP, & other pertinent data for correlation.
Smear for Gram’s staining
Smear on slide & gently heat-fix with a match or lighter. No fixative required.
Body fluids cytology (CSF, pericardial, pleural,peritoneal, synovial fluid, FNAB aspirates,etc,)
Immediately fix in 95% ethyl alcohol in 1:10-20 volume ratio (1 part specimen, 10-20 parts 95% ethyl alcohol) in a tightly sealed plastic bag or specimen bottle.

KOH Mount of Skin Scrapings
No fixative. Apply scraped tissue to slide, cover with another slide, and wrap in paper.

Biopsy
Small (FNAB, CNB), large, & radical specimens:
Fix in 10% buffered formalin in 1:10-20 volume ratio (1 part specimen:10-20 parts 10% buffered formalin) in a wide-mouth container

RADICAL SPECIMENS (MRM, etc)
To be fixed in formalin for at least 24 hours before histopath processing.

FROZEN SECTION SPECIMENS
NO FIXATIVE. Bring specimen immediately to processing laboratory. Results in 20-30 mins.
Can be arranged at the nearest affiliate hospital.
IMMUNOSTAINING (Immunochemistry/Immunocytochemistry/Tumor markers)
Various tests, e.g. ERA, PRA, HER2 neu, Cytokeratin, Vimentin, chromogranin, p63 or p40, CD3, CD20, CK7, CK20, Melan A, PAS
Microscopic slide and paraffin block of specimen required

SPECIMEN STORAGE
Small to large specimens shall be stored in formalin for seven working days only after release of official report, thereafter the specimen shall be discarded.
Radical specimens due to cancer shall be stored for 14 days only.
Slides & paraffin blocks of cases shall be stored for only 4 years from the date of request.
TURN-AROUND TIME
Computer-printed official reports will be available within the following time frames:
PAP’S smear, Tzanck’s smear, Gram’ staining and body fluid without Cell block : within 2-3 working days. Sundays & Holidays no processing.
Body fluid with cell block, small and medium biopsy – 3-4 working days
Large and radical specimens – 5-7 working days
Additional printed copy of results - 1-2 working days

Note:
The benchmark of some histopathological cases (e.g. carcinoma) is immunochemistry/immunostaining). If a case requires additional tests for confirmation of diagnosis, suggestion/outsourcing shall be recommended.
Provisional Diagnosis may be reported but Final diagnosis will be based on, or correlated with, the specific immunostaining test
COMPUTERIZED COPY OF REPORTS (DATA BANK) may be provided periodically upon request. External hard drive should be provided by the REFERRING LABORATORY.
BORROWING / PURCHASING OF SLIDES & PARAFFIN BLOCKS
2-3 working days (review of the case will be undertaken)
GROSS DOCUMENTATION: PHOTOGRAPHY & MICROPHOTOGRAPHY
A schedule may be arranged if gross photography is desired.
During the operation, the specimen should not have been dissected to the point that the morphology is obliterated.

Photos (gross and/or microscopic) may be requested for a minimum fee.
